Diacetylmorphine has had a long history since it was first synthesized by C.R.A. Wright of St Mary's hospital medical school, London in 1874, by reacting morphine with an excess of acetic anhydride. In Wright's original work, the substance was termed as tetraacetylmorphine. Later in 1898, the substance was marketed by the Bayer company of Elberfeld, Germany, with the name heroin.

Nowadays heroin is generally not used as a pharmaceutical. Due to it's high illicit production it is one of the most significant illicit drugs in terms of hospitalisation, overdose, drug related mortality, involvment of organised crime and drug related violence
